Healthcare Assistant

Job purpose

Duties and responsibilities 

  1. Assist in taking clients’ vital signs.

  2. Report to superior if any abnormality observed.

  3. Conduct activities with clients.

  4. Assist client in showering, ambulation/exercise, and toileting.

  5. Kitchen duties which involvesinvolve simple food preparation.

  6. Disinfect and clean centreCentre and equipment.

Enrolled Nurse (Day shift/Night shift)

Job purpose

  • To provide optimal level of care in accordance to established policies and procedures.

  • To assist in activities and programs promoting healthy aging for clients.

  • Supervise Nursing Team comprising of Nursing Aides and Healthcare Assistant to ensure optimal delivery of required care to the clients.

Duties and responsibilities 

  1. Comply with approved Nursing SOPs.

  2. Work with multidisciplinary team

  3. To analyze and evaluate nursing care plan and ensure care plan is implemented by team.

  4. Identify symptoms of clients related to illness or decline in functional/cognitive abilities and advocate early attention.

  5. Mentor and guide, NAs and HCAs in performing their functional duties.

  6. Administer medications and treatments as prescribed.

  7. Assist client to manage common age-related conditions such as incontinence and loss of mobility.

  8. Assist Registered Nurse as when required.

Nurse Manager

Position available at:

SASCO @ Hong San

Job purpose

The Jobholder oversees the care team comprising nurses, healthcare assistants and therapy aides, ensures delivery of quality, holistic, person-centred care to residents and oversee sheltered home operations. Jobholder will also be responsible for geriatric meal development, and management of pilot programme for 24x7 home care for seniors living in community duties

Duties and responsibilities 

  1. Supervise daily operation of the care team comprising nurses, healthcare assistants and
    therapy aides

  2. Assist Head, Centre Management to oversee the operations and management of the
    sheltered home

  3. Work with various stakeholders and other professionals such as MSF and Psychosocial
    Services (PSS) team, Community Engagement team for the holistic, person-centred
    management of residents, including development of appropriate care plans

  4. Jointly implement with PSS care plans and activities to support active ageing and
    improve residents' quality of life

  5. Develop and implement various guidelines pertaining to nursing and care delivery within
    the Home

  6. Continually seek out quality improvement opportunities in areas such as care and
    operations

  7. Educate and motivate care team to be aligned with organisation’s care philosophy

  8. Schedule care staff to attend training to ensure their competency and skills upgrading

  9. Recruit and conduct care staff appraisal

  10. Oversee adequacy of medical supplies/ equipment and ensure regular quality control
    check of medical equipment

  11. Work with experts to develop suitable geriatric meals for seniors, and with central
    kitchen and/or vendors for meal supply

  12. Develop and manage the pilot programme for 24x7 home care for seniors living in the
    community

Qualifications & Skills Required

  1. Diploma or degree in nursing and registered with Singapore Nursing Board 

  2. Previous experience working in an elderly care setting with person-centred care approach is an advantage

  3. Preferably at least 5 years’ experience in similar position, with eldercare and operations/administration responsibilities

Nursing Aide

Qualifications & Skills Required

  1. Minimum GCE ‘N’ Level

  2. Attained Training in either WSQ Higher Certificate/Advanced Certificate in Healthcare Support (Nursing Care); or Institute of Technical (ITE) Approved Training Centre (ATC) Healthcare (Home Care) program.

  3. For Foreign Trained : Degree in Nursing.

Social Worker

Position available at:

Job purpose

The jobholder will provide direct social work case management, counselling to clients and their families, conducting social and financial assessment. Develop casework intervention plans, implementation and evaluation of programmes in order to help clients adapt to their limitations and assisting families with important decisions about their loved ones. Lastly uphold professionalism in social work and SSCH’s care philosophy of person-centered care.

Duties and responsibilities 

  1. Review applications from AIC. Interview client’s and caregivers during enrolment into our programme/Home. Conducting Biopsychosocial assessment to assessing the strength and resilience of the client, families, and social support systems.

  2. Build rapport with residents/clients and their families. Maintain constant communication with clients while addressing their concerns and goals and helping them keep positive attitudes. And educating the family members on the physical and psychosocial needs of the clients and ways they can access internal and external resources.

  3. Provide casework management for our clients and coordination with external agencies such as hospitals and polyclinic for clients who require financial assistance for their medical appointments.

  4. Develop and implement individual care plans (ICP) for clients and provide therapeutic support as needed. – working with other multidisciplinary team (MDT) members.

  5. Execute the ICP and monitor progress. Develop and implement programmes and activities through a planned/structured manner that is aligned to SASCO values to meet the clients care needs.

  6. Coordinate with PT and OT departments to assist in helping clients feel comfortable and confident.

  7. Put up 6th monthly ICP review reports and monthly review records for clients.

  8. Evaluate existing, new and prospective clients based on their needs, desires and limitations.

  9. Update clients particulars and social report/review. Conduct casework evaluation, maintain case notes/documents.

  10. Person centred counselling for clients and/or families - counselling clients who suffer from poor mental health states (e.g. depression, anxiety), and coping and adjustment difficulties in their transition.

  11. Risk assessment - assessing risk of self-harm (e.g. suicide) and to others (e.g. family violence, elder abuse).

  12. Financial assessment and fund management and prepare periodic reports as required - identifying and referring cases for financial assistance (e.g. generate Means Test Subsidy Change Rate Report from MOHH for invoices printing/Changed Subsidy report, IHDC Master List updates, Quarterly consumable claim submission. SMF Consumable/devices application/renewal. SMF Consumable/devices purchase order)

  13. Any other additional tasks assigned by the centre manager.

Qualifications & Skills Required

  1. Degree in Social Work.

  2. Preferably with 1 – 2 years of relevant working experience

  3. Experience in Case Management, Mental Health and Dementia Care (Elderly/Mental Health/Community work).

  4. Experience in Intervention Therapies (e.g. Person-Centered Therapy)

  5. Strong administrative skills (in Microsoft Excel/Word/PowerPoint)

  6. Proficiency in Languages (English, Chinese/dialects, Malay) will be an advantage

  7. A passion in working with the elderly

  8. A good team player with pleasant personality and excellent interpersonal skills and ability to work independently

  9. Ability to relate and communicate with diverse population and groups

  10. Passionate about removing or reducing psychosocial-emotional, psychological and/or financial blocks that impede our clients from achieving a better quality of life

Therapy Aide

Position available at:

Job purpose

  • To support and assist the Physiotherapist and Occupational Therapist in carrying out the required therapy for the clients.

Duties and responsibilities 

  1. Conduct activities prescribed Physiotherapist and Occupational Therapist.

  2. Report clients’ rehabilitation progress to Physiotherapist and Occupational Therapist.

  3. Documentation of all activities conducted.

  4. Update monthly activity attendance.

  5. Display weekly activity schedule.

  6. Perform vital sign checks before conducting any activities.

  7. Assist in toileting when necessary.

  8. Maintain proper record and housekeeping of rehabilitation equipment.

  9. Other additional operational duties as assigned by Centre Manager.

Qualifications & Skills Required

  1. Diploma/ HMI certificate in a related field

  2. WSQ qualification in Therapy
